I've seen a lot of men ignore testicular problems in my career as a doctor. (Parents, educate your young sons to be aware of infertility and testicular loss from concealing painful testicles). Even so, I would never think of nominating most of these males for Darwin awards because of their lack of medical knowledge.

One patient, breaks this rule however, and especially as he was in his forties, seriously deserves to be nominated... He drove across the Australian countryside to have a routine operation that had been planned some months before at a city centre. The drive took him several days and on the way he passed several regional hospitals.

I had to do his admission when he arrived and I was rather curious when he didn't want me examining his abdomen or lower half. It turns out that a week or so before he had developed testicular pain which had worsened during the three day drive. Rather than seeking advice about his swollen, painful red testicles at some stage in the previous week he preferred to ignore the problem.

As a result, the operation was cancelled and new surgery replaced it. His testicles looked like discoloured grapefruits due to the severe infection he had somehow thought was going to be ok...and, sadly, as a result of the operation, he now has more room under him when he sits down. I still don't understand how he could drive so far on something so terribly painful...the price of denial.

...Swollen to the size of *grapefruits*!?! That doesn't require a whole lot of brains to realize something's very very wrong. And an infection that bad would result in fever, chills, other symptoms... It's a shame this has to be a PA!
