No media coverage so here goes. While working as a Foreman at a Bay Area refinery, some work was being done on one of the raised pipeways. My boss noticed that one of the contract welders had set up his oxy-acetylene bottles without securing them and told me to tell him to secure them. After doing so, I came back a while later and found the bottles arc-welded to one of the pipe stanchions. Needless to say, if he had burned through either bottle, we would have had a disaster on our hands. The contractor was immediately let go. Another near miss
