I have just pasted this from the company's own internal safety news sheet. If you give me an email address I can post the whole document to you. The machine in question is a like giant food mixer you can climb into. The "jog" feature will allow you to move the blades round 6" at a time by just pulsing the button. Seems he forgot which way they went.

Fatal Incident – Employee

Details of the Incident An employee died as a result of his injuries when he became entangled with the rotating blades of an ice crushing machine. The machine is used to crush ice, which is then added to the concrete mix design to help control the temperature of the material during very hot weather. The employee was part of a maintenance team who were carrying out preventive maintenance on the ice machine, which involved entering it to remove scale and apply lubricant to the inner walls. When the employee had completed the work inside the machine, instead of working his way back to the point he entered, he attempted to exit the machine through a small gap between the blades. When he realised the gap was too narrow, he reached for the “jog” switch, which allows partial rotation movement for the rotating blades, to widen the gap, but instead of moving in a direction away from him they moved towards him causing fatal injuries.

Yet another example of how laziness can be fatal. I'm surprised there wasn't some sort of safety interlock that prevented the machine from moving at all when a person was working inside it. Thanks for sharing this with us.
