A quite obvious and easy-to-follow safety precaution while operating a forklift is: Don't position your frail human form under the fork carriage. Us good forklift operators know that. In June 2005, a forklift operator in his late 50's from Oslo showed that he hadn't grasped this piece of wisdom when he, decided to investigate what seemed to be a hydraulic oil leak from his forklift. He raised the fork carriage, stepped out of his forklift and laid down on the ground under the carriage to try to find out where the oil was leaking from. He was incredibly successful at this venture when the fork carriage seconds slammed into the ground crushing him to death due to oil pressure loss because of the leak.

I'm not particularly amused by the write-up, but I'll give it a nod. It makes perfect sense that a hydraulic oil leak would affect anything hydraulic, including (and oerhaps especially) the lifting mechanism. I'm left wondering why he chose to investigate the leak himself instead of contacting someone whose job it is to do so...

Yup. I agree. I would think it would be common sense to not stand under the forks, never mind lie on the ground under them inspecting a hydraulic leak! It's got to be the forklifting profession's equivalent to striking a match to see where a gas leak is. Definite DA for my money. (Though could indeed use a better write-up.)

I'm sorry but while I think the incident is definately worthy an award, I can't agree to give it one without being able to verify it. You gave dagbladet no. 14 as the refrence. I don't even know what that is much less where to find it. Can you provide an online link or something so that I can verify the story?

It could use a funnier write-up, and I don't think I've seen this before. "Dagbladet" is Norwegian for Daily Press, I believe, so dagbladet.no is a newspaper web site, I think actually the big Oslo daily.
