A few months ago I got a job in a brewery. One of my first tasks was to copy up the hard reports of Industrial Accidents onto computer. Most of them were pretty pedestrian stuff, but there was one that stood out as DA material.
Two workers were repairing the hatch at the top of a fermentation tank, when one of them dropped a tool into the tank. One of the workers 'remembered' that these tanks had a ladder to allow people to climb inside and do maintenance. However, the tanks we use have very clearly marked pressure-sealed hatches at the bottom for maintenance. Still, the worker decided to squeeze inside (no small feat considering these hatches are quite small) and retrieve the tool. Unfortunately the lack of ladder caused him to fall about 15 feet into the fermenting alcohol. The coroner was unable to determine whether he died of drowning or alcohol poisoning.
