While stationed on an Aircraft Carrier, all are given safety lectures and demonstrations. In one particular case in 1990, a First Class petty officer assigned to the mess deck was briefing the new junior personnel on electrical safety, and periodic maintenance. He showed them how to fill out and put a warning tag on the circuits that you were turning off, in order to do maintenance. He then informed them that it forbidden to start work on the unit, until a separate person following behind, had checked the tag and circuit, to make sure the proper circuit had been secured. The verfier would then notify the tagger that it was okay to start work. However, giving a prime demonstration of why the rules were in place, and not waiting for the person to verify the tag, he proceeded to open and grab a circuit that was supposedly de energized, killing himself in front of 20 students.Submitted on 07/07/2002
