I am pretty certain this was the summer of 2000 and it can be verified through CKCO TV, Kitchener, Ontario Canada (part of the CTV Network), the Kitchener-Waterloo Record and I'm also pretty sure the Toronto Star newspapers. It may have been on the CTV National news also as it was well publicized.
On a farm near Kitchener, Ontario (forget the exact town name), several workers on a farm were using a manure spreader when it stopped working. In an attempt to repair it, one worker climbed inside the spreader.
After not emerging for several minutes, a second worker decided to see why and climbed inside also.
A short while later, after neither the first or second man came out, a third worker climbed inside to check on them.
A little later, a fourth worker became concerned because none came out and went to get help. All three who had entered the tank had suffocated within minutes of entering from the noxious gases inside the spreader.
