There is a small firework company in the SE of England called Skyhigh Pyrotechnics who produce pyro devices for the military and theatre and they also produce fireworks for the traditional november fifth celebrations. In late october 1998 (I think) two people, the police think it was two, decided to break into one of the explosive magazines with an oxyacetylene torch gun, presumably to steal some fireworks. At about one o'clock in the morning an explosion was heard up to five miles away and when the police arrived they found the foot thick reinforced roof blown some 20 feet away and all the walls flattened. The oxyacetylene torch was still there but the culprits got away. It was concluded that they had about five seconds warning before the big bang which just gave them time to get clear. (I am not sure if this qualifies because they survived)

It would need some kind of verification to be of usable quality.

A bit thin on detail, but using an oxyacetylene torch to cut into a firework store is astoundingly stupid.
